What kind of referral does Shirley accept?


Shirley accepts private referrals, clients with a NDIS plan (relevant support category), DVA (gold cards only), My Aged Care as well as patients referred under the Chronic Disease Management (formerly EPC plan)

Please note patients will incur an out-of-pocket fee for private and EPC referrals. 

How do I refer my patients or clients to Shirley?
  • For NDIS referrals:

    • Please fill in the referral form here

  • For DVA referrals:

    • Fax the D904 referral form to 02-9980 5323 and your patient/client can call 0491 020 128 for an appointment

  • All other referrals: 

    • Call Shirley directly on 0491 020 128 or 

    • Email your request to 

    • For patients using the Chronic Disease Management plan (MBS items 721 and 723), please fax the Medicare form to 02-9980 5323.


How do I know what Shirley discussed with my patient?


As the referrer, you will receive a written communication including a summary of assessment and intervention within two weeks of initial consultation. 

What are Shirley's consultation fees?

Please refer to the 'Services' tab.


All dietitian consultations incur out-of-pocket fees (except NDIS, DVA gold card holders with relevant D904 referral form). This practice does not offer bulk billing dietetics service.


Concessional rate can be arranged depending on the circumstances of the client.
