What types of referral does Shirley accept?
Shirley accept:
- Self-referral
- Medical referral from your healthcare provider e.g. GP or medical specialists
- Chronic Disease Management (formerly EPC plan)
- Client with a current NDIS plan with relevant support category - Plan-managed and self-managed NDIS plans
- Referral from case manager approved by Lifetime Care

Please note patients will incur an out-of-pocket fee for private and Chronic Disease Management Care Plan (formally EPC) referrals. Payment is required at the end of consultation via credit card or invoicing (direct deposit).
What are Shirley's consultation fees?
All appointment is charged at:
$ 190 for one-hour initial consultation
$ 95 for a 30-minutes review or
$140 for extended 45 minutes review

All dietitian consultations incur out-of-pocket fees (except NDIS, Lifetime Care and Breast cancer patients with a valid chronic disease management plan ).

Rebates
Once payment is made, you will receive a paid invoice via email. You can take this to Medicare or your private health fund (depending on level of cover) for rebate. Medicare rebate is $61.75 per session from 1 July 2025. You can arrange your Medicare rebate via Medicare Express phone App or via My Gov website.

Example- An initial 60 min consultation (under the Medicare chronic disease management scheme) of $190 would be followed by a rebate of $61.75 from 1 July. You will be ‘out of pocket’ $128.25 ($190-$61.75). A follow-up consultation of $90 would be followed with a rebate of $61.75, which means you will be $28.25 'out of pocket'.

Do you provide meal plans?
Many people ask me this, and the answer is: sometimes, but not always.
​
In the early stages, I may share a sample meal plan to show you how healthy eating strategies can look in everyday life. This can be a great starting point and a useful discussion tool.
​
That said, meal plans do have some downsides:

They can feel restrictive and don’t always fit the needs of others in the household;

Seasonal changes and food availability can make them hard to stick to;

Most people find them difficult to follow long-term.
​
Instead, my goal is to help you build the skills and confidence to plan meals yourself. This way, you can enjoy flexible, realistic, and sustainable eating habits that suit your lifestyle, food preferences, and nutrition needs.
For those who prefer more structure, I do offer personalised meal plans as an optional, fee-based service. Fees varies depends on the number of days required. This ensures you get both the guidance you need and the flexibility to make it work in real life.

Can dietitians sessions cost be claimed through my health fund?
Yes. You may be able to claim session cost with any major private health provider, depending on your level of cover. Contact your health fund for rebate amount and other details.
Dietitian item numbers are 500 (individual initial consultation) and 600 (individual follow up consultation). After each session, you will be provided with an receipt.
Please note: you cannot seek rebate with both Medicare and private health fund for the same appointment.
