For Referrers

What types of referral does Shirley accept?

Shirley accept:

- Private referrals

- Chronic Disease Management (formerly EPC plan)

- Plan-managed and self-managed NDIS plan with relevant support category

Please note patients will incur an out-of-pocket fee for private and Chronic Disease Management Care Plan (formally EPC) referrals. You may receive a Medicare rebate of $58.3 if your usage is within what your GP has prescribed through the care plan.  You will need to arrange Medicare rebate via Medicare Express phone App or via My Gov website.

What are Shirley's consultation fees?
 

Telehealth appointment is charged at:

 

 $ 190 for one-hour initial consultation

 $ 95  for a 30-minutes review

​

All dietitian consultations incur out-of-pocket fees (except NDIS). This practice is not a bulk billing dietetics service. Concessional rate can be arranged depending on the circumstances.

Can dietitians sessions be claimed through my health fund?

As Shirley is a Medicare provider and Dietitians Australia. You will be able to claim your session with any major private health provider, depending on your level of cover. Most health fund uses the Medicare Provider number as health practitioner identifier. Contact your health fund for rebate amount and other details. Dietitian item number are 500 (individual initial consultation) and 600 (individual follow up consultation). You will be provided with an receipt to help make this claim.
